Our Clinic Policies

Privacy Policy

At Spine & Sports Centre, your privacy is treated with the utmost care and confidentiality.

This privacy policy outlines how we collect, use, and protect your personal information in line with current State and Federal privacy legislation. We only collect and retain personal or communication information if you have chosen to receive care at our clinic.

What Information We Collect

To provide care, please share personal details, including your name, date of birth, address, medical history, and other relevant information. This helps us:

• Understand your specific health needs

• Allocate appropriate appointments

• Communicate effectively with you regarding your care

We may be able to reach out to you through phone, email, SMS, or mail. You can opt out of non-essential communication at any time.

Information Sharing

Many of our patients are referred by other medical or allied health practitioners. We may wish to correspond with your general practitioner or nominated healthcare provider with your express permission. This communication is intended for accredited professionals and contains only the necessary information: name, date of birth, and address.

If a third party, such as an insurer or legal representative, requires a report, we will only provide this once we have received a signed Release of Information document from you.

Confidentiality and Data Security

All information is stored securely and handled with great diligence. Our staff adhere to strict confidentiality protocols that apply during and beyond their time of employment at Spine & Sports Centre.

We may use secure dictation software during consultations to assist with accurate clinical note-taking. Please let your practitioner know when you visit if you prefer not to have your consultation recorded.

Policy Updates

Spine & Sports Centre reserves the right to update this privacy policy at any time. If you have any questions or concerns regarding your privacy, please don’t hesitate to contact us.

Cancellation Policy

At Spine & Sports Centre, we value your time and appreciate you respecting ours.

If you need to cancel or reschedule your appointment, we require at least 24 hours’ notice. This allows us to offer the time to other patients on our waiting list or those needing urgent care.

We understand that unexpected situations, such as illness or compassionate circumstances, can arise. In these cases, please get in touch with us as soon as possible.

• Cancellations with less than 24 hours’ notice or missed appointments will incur the full cost of the scheduled consultation.

• If more than 24 hours’ notice is provided, or if the cancellation is due to sudden illness or compassionate grounds, no cancellation fee will apply.

To reschedule, please call our team on (08) 9221 8458, and we’ll be happy to help.

You understand that sharing accurate and complete health information is essential for providing safe and effective care. Withholding or misrepresenting medical details may negatively impact your treatment outcomes.

You agree to:
